I was rather expecting a bit of some PowerPoint slides which might have contained more of the specifics of the outline. Some of our memebers were not able to be present so the outline view is not enough to catch them up and sometimes we wrote down what we thought we heard, and later in the Q & A realized that we had misconstrued the information. :)

Some information seemed to be contradictory both within the speaker's comments and with the information released during CMS open forum. In particular, CMS stressed expectation that generic notices would NOT be generated on admission except in rare instance where only a very short length of stay was expected. I will now be going back again to CMS for further clarification.

Headers on forms pages 3-4 were not visible. The difference between the Detailed Explanation of Non-Coverage and an Expedited Review Notice was unclear. I know new initatives from CMS are often not well thought out from an implementation point of view. It appears that we don't have adequate information to implement program in a timely manner. Not to mention the fact that a law suit is driving up health care costs for all providers.

Ms. Hogue assumed that all participants had an intimate grasp of the concepts to begin with. She should have spent 10 minutes outlining the BASICS of the "Current" HHABN and Expedited Review/Detailed Explanation forms and their intended use BEFORE going into the proposed changes and the weird case scenarios. Let's get a basic grasp of the proper intended use, and then build on that to what needs to happen beginning July 1st. I kept getting lost in her explanations, and found the participant questions often mirrored my own and provided better clarity. Thank you!

Some of the comments made my Ms. Hogue seemed to contraindicate what CMS said in their open door forum on Monday June 20. Others who listened to the conference with me felt that, based on what Ms. Hogue stated, that we would not need to do these at discharge because Medicare benefits were not terminated. They interpretted that since Medicare would pay for another episode of home helath if needed, that we didn't need to utilzed the expitied determination form.
